We are excited to announce our upgrade to a fully functional EMR (electronic medical record) software including a new Patient Portal.

If you have an upcoming appointment, you MUST register and create an account with Patient Portal in order to be seen by a provider. Be on the lookout for an email and text message with instructions to set up your profile. Email will come from mypatientvisit.com and text from 813-710-4634.

IMPORTANT: You will be required to complete your intake forms, including medical history, utilizing the patient portal. Your information will immediately be available to the office staff and Providers to better prepare for and conduct your visit. We will no longer be able to accept paper intake documents.

Your Visit Begins Before Your Provider Sees You. The information you provide during the Patient Portal intake process is very important and helps frame your visit experience. Our providers are investigative and review your health history in detail prior to your visit. There are many reasons why specific questions are asked. Answering them in detail provides crucial pieces to your overall health history. Take this process and yourself seriously. The entire Patient Portal intake can take 20-30+ minutes depending on your history.
