FreshSkin Practice Policies
CONSIDERATION: We take great pride in providing a serene and safe environment for our patients and guests. This is your time. To respect others and for the safety of all, we ask that you please leave children at home. As a medical facility we have equipment and tools that are not child-safe, and therefore is not a favorable environment for little ones. While we recognize that everyone has busy lives, and mobile phones are an important part of our lives, we ask that you be respectful of all of the other patients having services. When you arrive for your appointment we ask that you please turn off all cell phones and mobile devices. In addition, we ask that you keep the noise level at a minimum.
OUR SKIN CONSULTATION: We are absolutely committed to helping you get the skin that you deserve. Because of this commitment, we offer complimentary consultations utilizing the VISIA® Complexion Analysis system to assess the quality and condition of your skin. This analysis gives us the starting template that will help us plan and prescribe a skin care program that is specifically tailored to your individual needs. This template then allows us to accurately monitor how well your treatments and skin care products are working for you. Please be prepared to have your makeup removed, and bring in all of your at home skincare products.
COMPLIMENTARY WEIGHT LOSS CONSULTATION: Weight loss can be very complex and troubling. We offer a complimentary 15 minute consultation to review your options. If you have a complex medical history, we recommend booking a wellness consultation as these issues cannot be addressed in the complimentary consultation.
COMBINING WELLNESS AND COSMETIC APPOINTMENTS:
In order to provide the best possible care, we cannot combine wellness concerns with aesthetic (cosmetic) concerns in one visit. Attempting to combine these visits into one runs the risk of incomplete documentation, and other forms of human error and may jeopardize the quality of the care you deserve. In the event that time allows and it is critical that you are seen for both visits, the provider will notify the front desk to add in a separate visit and you will be charged appropriately for that visit.
SCHEDULING: Due to the busy nature of our Practice please know it may take a couple weeks or more to see the Provider of your choice, so please schedule in advance. If your customized aesthetic or wellness plan includes follow-up visits or treatments to be completed within a specific timeframe, we recommend scheduling all appointments in advance as well.
NEW PATIENTS: Please arrive 15 minutes prior to your scheduled appointment time to complete the new patient intake forms to avoid cutting into the time with your provider.
CHECKING-IN: Please arrive 10 minutes prior to your scheduled appointment to prepare for your treatment. Because FreshSkin is a medical facility, we will require you to review our HIPAA policy and sign a release form.
LATE ARRIVALS: Our scheduling allows the correct amount of time to complete each service. If you are unable to arrive on time for your appointment please call our office and know you may run the risk of missing your treatment. We will do our best to see you, however it may be necessary to reschedule. If you arrive 15 minutes (or more) late for your scheduled appointment it will be considered a cancellation and a $75 fee will be assessed.
CANCELLATIONS: If you must cancel or reschedule your appointment, we require a 24-hour notice. All services rescheduled, cancelled, or missed on same day of appointment will be assessed a $75 fee.
WAIT LIST: If you would like a specific date or timeframe that is already reserved by other patients, let us know and we will be happy to add you to our Wait List. Please provide us with the best phone number to reach you and specify if you would like a call.
FINANCIAL POLICY:
Insurance: Although our office does not accept health insurance, many of our labs will bill your insurance. If you are having lab-work done and your appointment is medical in nature, please provide a copy of your medical insurance card. Upon request we are happy to provide you with a statement to provide to your insurance carrier for reimbursement.
Payment is due in full at the time of service. For your convenience we accept all credit cards, care credit and cash. All sales are final.
PRODUCT RETURNS: All retail purchases are final sale. Retail Skincare and Makeup Products may be returned/exchanged due to a documented skin reaction within 30 days from the date purchased.
If reaction of a product occurs please contact our office at 847-681-8821 immediately to schedule a complimentary follow-up with one of our providers.
GRATUITIES: Since this is a medical facility we do not accept gratuities. The greatest form of thanks is a referral or a review on Google, Yahoo or Yelp. We appreciate it!
